Il mwcg gabapentin cite-ref-2[2] è il nome del principio attivo indicato specificamente nel mwdwdolore post-erpetico e nell'mweaepilessia parziale resistente alle terapie standard.

È un farmaco mweganalgesico e mwewanticonvulsivante. Viene usato mwfamwfqoff-label per il mwfgdolore neuropatico, i mwfwdisturbi del movimento, la mwgafibromialgia (non approvato ufficialmente dalla mwgqFDA a differenza del mwggpregabalin), e altre problematiche. Viene commercializzato sotto vari nomi, tra cui mwgwNeurontin (mwhaPfizer). È un mwhqfarmaco generico dal 2004.

Il gabapentin si lega ai mwiqcanali del calcio voltaggio dipendente diminuendo l'attività di numerosi neurotrasmettitori come mwigglutammato, mwiwnoradrenalina, mwjasostanza P ed amplificando l'attività del neurotrasmettitore mwjqGABA aumentandone la concentrazione intersinaptica. È attivo inoltre sull'attività GABAergica non sinaptica.

Si usa in una vasta gamma di indicazioni; ma le indicazioni approvate sono: (Citato l'RCPcite-ref-3[3]):

* terapia adiuvante per l'mwlqepilessia parziale con o senza episodi di mwlgconvulsioni secondarie generalizzate in pazienti refrattari agli antiepilettici standard

* trattamento sintomatico delle mwmanevralgie posterpetichecite-ref-mordarski-2009-4-0[4]cite-ref-fleckenstein-2009-5-0[5]cite-ref-chou-2009-6-0[6]cite-ref-sasaki-2008-7-0[7]cite-ref-gilron-2005-8-0[8]cite-ref-rowbotham-1998-9-0[9].

Il mwsgdolore neuropatico in generale, di cui la mwswnevralgia posterpetica è una specifica istanza, non rientra tra le indicazioni esplicitamente approvate dall'mwtaAIFA per il gabapentin.

È inoltre curabile il dolore neuropatico periferico e mwtgmialgico causato da complicanze dell'mwtwalgodistrofia (detta anche mwuadistrofia simpatica riflessa o mwuqsindrome dolorosa regionale complessacite-ref-10[10])

In una Cochrane mwhwreview del 2005 si conferma l'utilità del gabapentin nel dolore neuropatico, ma l'efficacia dello stesso nel dolore acuto resta da documentare megliocite-ref-wiffen-2005-19-0[19].

• alcuni psichiatri usano prescriverlo per il trattamento del mwarsdisturbo depressivo e del mwarwdisturbo bipolarecite-ref-vedula-2009-34-1[34]cite-ref-mack-37-1[37]cite-ref-cascade-2008-48-0[48]cite-ref-moreno-2004-49-0[49]cite-ref-spina-2004-50-0[50], come mwatestabilizzatore d'umorecite-ref-lovell-1999-51-0[51]cite-ref-freeman-1998-52-0[52], in sostituzione di terapie con maggiori effetti collaterali; allo stato attuale tale uso appare controversocite-ref-evid-53-0[53]. Ma studi recenti hanno riscontrato che il gabapentin non è più efficace del mwat4placebo ed in uno studio addirittura risulta essere meno efficacecite-ref-54[54]

• mwauqdolore cronico, mwauureumatico e mwauyarticolare, mwaucsciatalgia, mwaugradicolite, dolore da mwauksindrome da compressione nervosa, mwauofibromialgiacite-ref-55[55]

Uno studio del 1999 valuta l'uso del gabapentin nel mwavabruxismocite-ref-brown-1999-56-0[56].

mwavcIpersensibilità a gabapentin (principio attivo) o a qualsiasi altro eccipientecite-ref-57[57].

• mwav8Epilessia, 300mwawa mg una volta al giorno, per i primi giorni poi adeguare il dosaggio a seconda del caso

• mwawiDolore neuropatico, 300mwawm mg al giorno (dose massima 3,6 g al giorno)

mwawySuicidio/ideazione di suicidio: l'assunzione di farmaci antiepilettici, tra cui gabapentin, per il trattamento di crisi convulsive, disordini psichiatrici, mwawcdolore neuropatico o altri disturbi può determinare un aumento del rischio di mwawgsuicidio nei pazienti o l'insorgere di idee e comportamenti suicidari. Il confronto con il topiramato ha evidenziato un aumento del rischio di suicidio associato a gabapentin (hazard ratio 1,42). Monitorare eventuali segni o sintomi riconducibili a idee suicidarie; in base alla gravità dei sintomi può essere necessario ridurre o sospendere l'assunzione di gabapentincite-ref-58[58]cite-ref-59[59].

mwaxiInterazioni con morfina: l'assunzione di gabapentin in seguito a somministrazione di mwaxmmorfina a rilascio prolungato determina un aumento della concentrazione plasmatica del farmaco antiepilettico. Monitorare il paziente e, in caso di insorgenza di effetti collaterali a livello del mwaxqsistema nervoso centrale, ridurre opportunamente le dosi di gabapentin e di morfinacite-ref-60[60].

mwaxoEpatite: in letteratura sono riportati casi di mwaxsepatite e mwaxwittero colestatico associati al trattamento con gabapentincite-ref-61[61]cite-ref-62[62]cite-ref-63[63]cite-ref-64[64]. Si consiglia di monitorare i valori di ALT e AST sia prima che durante la terapia per individuare un eventuale danno a livello epatico

mway4Disturbi della vista: in letteratura è riportata una segnalazione di restrizione del mway8campo visivo associata a terapia con gabapentin, reversibile con la sospensione del farmacocite-ref-65[65]. In caso di insorgenza di gravi disturbi della vista, ridurre il dosaggio di gabapentin o sospendere la somministrazione del farmaco

mwazumwazyDisordini del movimento: in letteratura sono riportate due segnalazioni di disordini del movimento associati a gabapentin, che si sono risolti in seguito ad interruzione della terapiacite-ref-66[66].

mwazwMorte improvvisa: nel corso degli studi clinici pre-marketing si sono verificati alcuni episodi di mwaz0morte cardiaca improvvisa tra i pazienti in trattamento per l'epilessia con gabapentin. L'incidenza di tali eventi è risultata essere simile all'incidenza riscontrata sia nei pazienti in terapia con altri farmaci mwaz4anticonvulsivanti, che nei pazienti epilettici non trattaticite-ref-67[67]. In questi casi non è stata comunque stabilita una relazione diretta di causalità con il trattamento a base di gabapentin.cite-ref-68[68]

mwaagInterazioni con l'allattamento: sono disponibili dati limitati riguardanti l'assunzione di gabapentin durante l'allattamento. La quantità di gabapentin presente nel latte materno risulta essere minore rispetto alla quantità di farmaco a cui il neonato viene esposto durante la gravidanza. La dose di gabapentin assunta dal bambino durante l'allattamento non sembra causare effetti avversi. La terapia antiepilettica nella madre non può essere sospesa per evitare l'insorgenza di fenomeni convulsivi; si raccomanda quindi di assumere la minima dose efficace di gabapentin per evitare fenomeni di accumulo nel bambino, poiché l'eliminazione del farmaco per via renale risulta minore rispetto ai pazienti adulti. Se possibile preferire l'assunzione di gabapentin in monoterapia. Si raccomanda comunque il monitoraggio di segni e/o sintomi riconducibili ad una eventuale tossicità del farmacocite-ref-69[69]cite-ref-70[70].

Sconsigliato l'uso prolungato per gli effetti avversi e in soggetti con mwabminsufficienza renale e mwabqdiabete mellito. Per quanto riguarda la somministrazione agli anziani questa può avvenire con dosaggi ridotti. Fra gli effetti collaterali si sono riscontrati anche diversi effetti paradossi. Tra gli effetti indesiderati: mwabusonnolenza (comune), mwabycefalea, mwabcnausea, mwabgvertigini, aumento di peso, mwabodisartrie, mwabsartralgie, mwabwdiplopia, mwab0parestesie, mwab4mialgia, mwab8dolore toracico, mwacaporpora, mwaceatassia, mwaciamnesia, mwacmastenia, mwacqpancreatite, mwacutrombocitopenia, mwacyimpotenza, mwaccleucopenia, mwacgdispepsia, mwackvomito, mwacodispnea.

mwac0Fenobarbital, mwac4fenitoina, mwac8acido valproico, mwadatramadolo o mwadecarbamazepina non interagiscono con il gabapentin; inoltre, gabapentin non influenza i contraccettivi a base di mwadinoretisterone e/o mwadmetinilestradiolo. Non è influenzato, inoltre, l'assorbimento del farmaco dal cibo. Per tutti questi motivi il gabapentin è molto maneggevole nell'uso.
